Our Impact
Integrated chronic care manages a broad range of chronic conditions, including both HIV and non-communicable diseases (NCDs). For NCDs, it focuses on less complex conditions like hypertension and type 2 diabetes, along with basic mental health conditions.

This is a primary care program where clients with different chronic diseases are in the same queue and seen at the same time by the same medical personnel. This model of care delivery streamlines care for clients with more than one chronic condition.

This integrated approach not only improves efficiency but also helps to reduce stigma and discrimination among clients within the program and the broader community. Integrated chronic care manages a broad range of chronic conditions, including both HIV and non-communicable diseases (NCDs).
For NCDs, it focuses on less complex conditions like hypertension and type 2 diabetes, along with basic mental health conditions. PEN-plus (Advanced NCD clinic), on the other hand, specializes in handling more complex NCDs. These include conditions like type 1 diabetes, heart failure, sickle cell disease, stroke, chronic kidney disease, and chronic liver diseases.
IC3 provides treatment and care for non-communicable diseases and HIV/TB clients. Conditions that are seen at IC3 include hypertension, diabetes type 2, asthma, Chronic Obstructive Pulmonary Disease (COPD), epilepsy, mental illness conditions, mood affective disorder, and advanced HIV diseases.
IC3 services are provided on different days in all 14 facilities with the same teams from Neno DHO and Lisungwi Community Hospital. The clinic has different cadres including clinical officers, nurses, clerks, data clerks, pharmacy assistants, and expert clients who work as a team to conduct clinics at all 14 facilities in Neno district.
